Send two documents:
- a current registry extract, such as an up-to-date printout or PDF from your official register
- your cooperative statutes, which set out the founding rules of the cooperative
Together, these documents should provide the information we need to verify your cooperative or mutual.
What information do your documents need to show?
Together, the registry extract and cooperative statutes should show:
- the cooperative's legal name, registration number, and registered address
- that the cooperative is still active
- the full name of every current board member
- who the members are and what each member holds, such as a membership share, percentage, or voting rights
If available, it also helps if the registry extract shows:
- the registration date
- country of registration
- tax ID
You can still continue with your application if these additional details aren't shown.
What if some information is missing?
If the registry extract and cooperative statutes don't show everything listed above, add the document that provides the missing information. You don't need to send all of these.
- Official list of board members filed with the registry: a board list that has been submitted to and accepted by an official body
- Resolution appointing the current board: the decision appointing your current board
- Register of members: the list of members and the share each one holds
- Annual return or annual filing: your most recent yearly filing, if it lists the board or members
- Certificate of incorporation or registration: the certificate issued when the cooperative was first registered
- Certificate of good standing or existence: confirmation from the registry that the cooperative is active and up to date with its filings
- Official confirmation letter from the registry: a letter or electronic confirmation for countries where the registry doesn't issue formal extracts
- Ultimate beneficial owner (UBO) declaration or beneficial ownership register extract: identifies the people who ultimately control the cooperative. This can support your other documents, but it can't replace them.
Does my country change which documents I need?
No. The same document requirements apply wherever your cooperative is registered.
What should you check before uploading your documents?
- Send the most recent version. We can't accept historical or superseded documents.
- Include every page, even if a page only contains a stamp or signature.
- Make sure the cooperative's full legal name and registration number are clearly readable.
- You can upload a clear scan, photo, or downloaded PDF. Make sure all text is legible.
- Documents in your local language are accepted. You don't need to translate them.
What happens after you upload your documents?
Your documents are sent for review after you upload them.
If we need another document or more information, we'll let you know.
